THIS ROLE IS LOCATED ALONG THE OREGON COAST We are hiring IMMEDIATELY for a DEVELOPMENTAL DISABILITIES SERVICES COORDINATOR to join one of our community mental health providers!
Summary:
We are seeking a Bilingual Required Spanish/English Developmental Disabilities (DD) Services Coordinator to provide case management services to those experiencing I/DD.
Duties:
Assist individuals and families in identifying appropriate community based and other resources, and in linking with those resources Perform protective services for individuals and comply with OAR requirements for documenting and reporting Provide outreach to the community to assure potentially eligible individuals are aware of services Assure there is an Annual Plan for each individual enrolled in services.
Attend the annual plan meeting and participate in the development of the plan for individuals enrolled in comprehensive services.
Develop the annual plan for all those not enrolled in a comprehensive service using the forms as provided by DHS Coordinate individuals plan to access home and community based services Monitor all services and supports on each Annual Plan.
Ensure that all Medicaid Waivered services are being provided according to the plan and OAR Coordinate services with other community agencies and providers Required Education and Experience Bilingual - English/Spanish required Bachelor's degree in behavior science, social science or a closely related field; or a bachelor's degree in any field and one year of human services related experience OR Associate's degree in a behavioral science, social science, or a closely related field and two years human services related experience OR Three years of human services related experience.
